Choose the appropriate waiver form below. Fill it in (it’s a fillable pdf), print it out and then sign the form. Then hand this form to your driver on the date you have booked. Each form is good for one year.

*If you’re unable to print, please tell us in the “MESSAGE” section of the Ride Request Form and our drivers will be sure to bring copies for you to sign.
